Office Policies
I look forward to seeing you on your scheduled appointment date. My office will send an SMS reminder prior to your appointment to confirm the appointment date and time.
Please complete the New Patient Form downloadable from the website and bring to your first appointment.
If you have seen Dr. Van Schenck in the past 3 years you are considered an existing patient but must still complete the New Patient Form and bring to your first appointment.

Appointments & Cancellations
Once you register for your appointment it is your responsibility to remember your appointment and arrive on time. All appointments are non-refundable except through proper cancellation through email or text by 12pm three days prior to your scheduled appointment (Ex: If cancelling a Thursday appointment, you must cancel by 12pm on Monday). If you cannot attend your appointment, please notify Dr. Van Schenck by voice message with details of the cancellation.
Telemedicine
For your safety during inclement weather or due to physical and travel difficulties I offer telehealth services for follow up visits. If you would like Telemedicine please let me know at your first appointment or request telehealth over the phone at least 48 hours prior to your appointment. I will call you to arrange.
Emergencies
Psychiatric Emergencies are:
- Suicidal thoughts or thoughts of harming others
- An unexpected medication reaction with serious symptoms
- An unusual behavior that you fear may lead to physical harm
* In case of emergency, please call 911 or go to the emergency department.
* Prescription refills are not considered an emergency.

All appointments are pre-paid online and payment is through online booking redirected to Square. For your convenience, we accept the following credit cards:
